Hi all
when I did setup survey site for deciding default theme for 3.4, I've also got idea to get more feedback from our users. This can help us in making some decisions about future development.
We can take inspiration from Git User's Survey [1] (check last survey and results [2]).
I think the survey should be open for pretty long time, let's say two months, to acquire as much feedback as possible.
I've started collecting questions on wiki [3], basically starting with questions based on the Git ones.

I think 5, 6 and 8 should have one more option - "I don't know" (and most questions should get some comment box).
14. Most people won't know what HTML5 is, it's better to add some explanation, or skip this altogether. HTML 5 availability is based on minimum supported browser versions, users won't care. Software should work :)
15. Add "never used any"
Otherwise looks ok. After completing questions I think we should give this survey to some phpMyAdmin beginner and ask for comments to find potentially difficult questions (ones that result in user quitting the survey). Also, I don't think we should go over 20 questions. If we have more, we should decide which ones to use in the actual survey.

Some Suggestions:
@Question 1,2: Interesting to know but how does that help for the future development?
@ Question 6,7 Some people might use phpmyadmin e.g. for MariaDB alone, so question 6 probably should probably be made dependent on question 7, whereas question 7 maybe should be changed into "For which Database do you use phpMyAdmin"
@ Question 8 & 9 'proficiency' sound complicated, how about 'knowledge'?
@ Question 14 Maybe this question should be changed into something like "Does your browser support the following technologies and have you enabled them?"
Plus some side note which major browser versions support html5 properly.

How about moving these to the bottom of the list and making the optional.
How about: "What features you desperately need to manage your DB, but are not available in phpMyAdmin?"? Well, maybe worded a bit better and with a textarea for the feedback. I'd say we won't get much response here, as people hate writing in surveys and prefer to only tick boxes, but we might get one or two gems...

Basically, we have the feature tracker for that, but maybe some users don't find the way to it. ;)

I believe that regarding surveys, the shorter the better.
Questions I would remove: ------------------------ What country do you live in (country of residence)?
How old are you (in years)?
Which phpMyAdmin version(s) are you using? (because we know that host providers and distros do not tend to upgrade quickly; knowing what's the version used by the persons answering the survey won't give us the full picture)
Which MySQL version(s) are you using? (I'm not sure what will be the benefit of knowing this)
Which PHP version(s) do you use to run phpMyAdmin? (because it might be difficult for them to answer)

I think it's good to know some basic information about our users.
Which phpMyAdmin version(s) are you using? (because we know that host providers and distros do not tend to upgrade quickly; knowing what's the version used by the persons answering the survey won't give us the full picture)
You can usually install own copy of phpMyAdmin, so this still gives us the information how much are people willing to be up to date.
Which MySQL version(s) are you using? (I'm not sure what will be the benefit of knowing this)
To see how many people can use features introduced in new MySQL versions.
Which PHP version(s) do you use to run phpMyAdmin? (because it might be difficult for them to answer)
Indeed it might be.

I find interesting to find out whether phpMyAdmin is mostly used by novices or by people who have great knowledge of SQL, but are lazy to write it :-).

For me personally, when a person or a website requests me to submit a survey where I have to give away personal information such as my age or country of residence it creates a small barrier for me to do so.

Okay, you both convinced me, I've removed the questions from the proposal. I've also simplified question about used server.
Any further comments?

Ok for me.
