For $Id, I think it can be anywhere but we tend to place it at top to 
see it better.

For the vim options, they say "at the beginning and end of file"; I 
experimented and it must be in the first or last 5 lines.

Sorry, I had not seen it :) because I looked only at the top. Yeah we 
should be more consistent on this.
