In particular, you really want to put in your proposal links to our Patches tracker where you submitted phpMyAdmin code modifications. Remember, we cannot guess your SourceForge.net user name so links will be clearer.

I have provided couple patches to PMA. So maybe it would be better to provide a link to my application draft in the mailing list where I discussed ideas. I would provide all the links to my patches in my application. Is that reasonable?

Yes, please list it in open source contributions as specified in the template. I've just updated the application template to provide explicit question about contributions to phpMyAdmin.
