Yes, indeed most stats are from Ohloh and a few (number of translations, etc) from phpmyadmin.net, with download numbers from sourceforge.net. I believe Ohloh licenses their data under Creative Commons Attribution 3.0 Unported (http://creativecommons.org/licenses/by/3.0) which means I should add attribution to the page, but I haven't finished looking in to whether it's their CODE that is so licensed or the DATA, which might matter before releasing this.

I would leave out the "if not the de facto standard" part. I wouldn't make such a statement, unless I was quoting someone.

To answer your question, I'm not sure about setting a place to receive birthday comments. We could invite users to add their reviews and possibly birthday wishes at http://sourceforge.net/projects/phpmyadmin/#reviews

That's an excellent suggestion. I'll add that and the Ohloh references later (unless someone else does it first) and send out the revised version.

Very well written! Small remark : should we mention that 69 is the number of languages is being translated into, but that the number of translated languages that are complete enough to ship with phpMyAdmin, is slightly smaller?
